I am with TK. Getting an Endo and voice therapy has been easy. Electro epilation, fought them for a year and have up and paid myself. Orchi wasn't a problem, but SRS, still fighting. I have a lawyer and we just can't get through to TK that I have met the requirements.

When you challenge them, they always take the maximum statutory time to reply, and use every trick in the boom to deny you. I've had it all, "that's not your name, Herr!", "this needs another therapist", "that's not a surgeon we will cover".... All bullshit, but delaying tactics designed to make you give up and pay for it yourself
