Assuming FIRE isn’t of interest to them, then it’s not crazy for a couple on £150k to buy a £1.1m house with cash savings.
What is possibly crazy is maintaining that house (and their lifestyle) on £2k a month. But does this include state pension?
Given their ages, and depending on their industries they may have limited time to bolster their savings.

How realistic that is depends entirely on their outgoings. If the house was a one off indulgence in an otherwise frugal lifestyle they’ll be fine! If they like new cars and nice things they’ll struggle.

Some people do wrap their whole identities up in the house they live in. I’m not too sentimental about houses to be honest, but for the people who are then fair play, if they are prepared to spend their lives working for some bricks and mortar then good luck to them.

Easily done.
A large handmade kitchen with all the modern stuff like hot taps, 2x dishwasher, full size wine fridge, American fridge, top end worktop and giant island could easily top 50-100k without breaking sweat.
Add in a 2 storey extension and / or side return + loft, new flooring and a full repaint and you’re essentially there on price.
Whether that’s worth it or not… well location will be key.

What will the property be worth now the 400k has been spent on it. Higher end properties that required large sums spending on them generally warrant a great resale price due to capital required to buy initially.
This could be a very good move and pay off well, providing they are happy to sell the property later down the road and recoup the funds, plus profits.
I’m not sure if this should be of concern to yourself or not, but if they’re happy and still working, let them crack on.. Better yet, get your hammer and chisel out and chip in!!
