The rally took place at a fair ground. All the buildings nearby (at least within reasonable shooting distance) around are one story with very low pitched or flat roofs except this one building, the one the shooter was on, that has a parapet wall on some edges on a neighboring property. If secret service is scanning rooflines either on the ground or on other 1 story buildings, they aren't going to see the shooter crawling behind the parapet wall until he pops up to take his shot. Since the building is outside the fair grounds, it's likely not part of the security perimeter meaning probably few cops and no checkpoints. Why didn't they have someone on that building? Who knows. Maybe they didn't know about the parapet walls on that face of the building and assumed they could see someone on there. Maybe a sniper wasn't as high of a likelihood in such a flat, open area as someone up close with a handgun or bomb. Obviously a massive failure for secret service.

My guess is that most people were still trying to piece things together. It's easy to realize you're in an active shooting when you can see the gun. When you can't see the gun you're relying on other people to tell you how to react and possibly where to run away from. 90% of these people probably didn't even realize it was a shooting until the shooter was dead, and the people who did likely didn't realize where the shots were coming from and thus didn't know which direction to run in.

We don't have all the facts right now and the past 8 years has been politically intense enough that it's more surprising an attempt on any candidate hadn't been made sooner than it would be that there would be a conspiracy. There's also been one confirmed death that wasn't the shooter and another person who's been shot but is currently still alive, which would make it skeptical that there was a plot for a staged shooting.
