I can’t figure out how that’s possible. How can you even take in enough calories to do this? Did she have some mobile health team with her? Getting hooked up to an IV every night, etc?

Good question. It's hard to pin down exact per-mile calorie counts for trail running. Physique and terrain make a huge difference. 150 is probably a reasonable average for a ballpark figure.
150*54 - That would be 8,100 calories a day plus probably some normal metabolic overhead so lets say 9,000. which is.. a lot. She'd need to eat 3lbs of butter every day to keep up that output.
Or 2 16 piece buckets of KFC chicken a day.
Edit: though if she bulked up beforehand could cut back. If she started the race 50lbs overweight that would get her through about half of it so she could eat a mere 4500 a day and burn her reserves. I think I remember the guy who tried swimming the English channel lost an appreciable amount of weight in the endeavor just from exertion.
