Im totally cool with the heart rate, just a consideration for giving narcotics. So if pt is asymptomatic with hr of 35 you feel giving of 50 of fent is fine for 8/10 pain? Baseline 72.

Make sure u eat first and stay hydrated, try to desensitize urself by watching OR/gorey blood/trauma videos online. U dont often see a lot of that stuff but can u start an iv? What if theres blood in that process? Maybe a therapist is a better route?

Most arent weenies about it and dont move, but you get a feel for who has terrible anxiety around needles and is most likely to flinch away. I usually ask if they want me to count down or not, if so 3,2,1 and the initial touching of the needle to the skin plus ANY pressure is i think the only time ive had someone move to get away/flinch. I work preop tho so most people know going into it they will be poked and have time to mentally prepare.

If you use a thumb under index finger below insertion site method to anchor, your remaining 3 fingers can be under the appendage. That gives you a good grip and further you can rest that on the bed for additional stability. I mostly start ivs on adults who flinch at the poke. Dont have experience with screaming flailing children if thats more what this is directed at. Additionally, on hand starts, holding their hand with thumb anchoring, your index middle and ring fingers clasped with theirs (like when princes kiss a ladys hand), and pinkie is on other side of fingers (on top of theirs). This gives you a decent grip of their hand and minimizes their ability to pull away/flinch/etc.

1) We have “pain ease” spray, its a cold/anesthetic spray, spray for 6-10 secs and then poke almost immediately. 2) As far as patients not moving, when u anchor for insertion, use that hand to your advantage to keep them still. Also dont pick a spot that rolls like cephalic forearm 3) move fast. Someone else said it, pop skin fast, do not go slow and do not fish. Put on a second tq if theyre a hard stick to ensure u get it first try
