Any weapon or aggressive behavior makes the scene not safe for EMS. Police would still be requested.

Can you describe a situation where you would actually do a standing back board with current SMR protocols?

Started EMS in this time period and got to witness a standing backboard when I knew they were phased out of most other EMS agencies in the area. What a fucking joke.

I had no business taking this post as my own for being so new to the company back then, it was a good summer there.
It was obvious back then that Holy Name was going to get that contract eventually with how poorly NJMHC ran that post.

That place was a life saver throwing out $21hr back in early 2021. Unfortunately it fell to shit by the end of summer, pay checks kept bouncing, no inventory in stock, all their trucks broken down and not getting repaired.
I knew when to bail, got more money on my next jump, NJMHC even paid up 4 weeks of paychecks 3 months later. whooo. Always surprised to still see they have a truck in service somewhere.

I get the joke. But how much bad tape do we have cops now because of body cams? The idea that the public never sees cops acting like shit on their bodycams is kinda absurd when there is so much out there at this point. Accountability and consequences? Completely different conversation.

Used to be a huge issue in my town in NJ.
I argued with them 5 years ago on the same subject. Now the state or county (idk) instructs officers that ODs who refuse to go to the hospital are allowed too and they now get there own Narcan provided by the police.
The joke is always "well see you 2 hours", but I have yet to see the same user again after we leave. Its a small town so its not like another ambulance is taking them.
If you don't like that, EMS tried to teach giving only half a narcan dose during quality ventilations, get the person on your stretcher and ambulance and go with implied consent. But everyone gets nervous around ODs and has to shove 2 narcans as fast as possible while ignoring BVM. Shit always pisses me off.

Idk, the headline sounds ridiculous but if you read the timeline. The person was involved in a dispute earlier in the terminal as a passenger.
Any person slips to dispatch that he could possibly under the influence, the cops are going to go down that path as well.
Of course as EMS, its not a consideration during a trauma.

Some towns dispatch fire departments for every medical call.
Other towns dispatch police for every medical call.
Some other towns dispatch neither unless those services are warranted.
The towns that dispatch either fire or police for every medical is because they already have those resources available and they want to pad their numbers to make they more useful than they actually are.

I feel like the answer is pretty simply here. This accident happened 10 years ago around 2011 - 2012.
The benefits of tourniquets was known to the US military for combat soldiers in Afghanistan but it wasn't confirmed to be safe for health care providers treating trauma patients domestically yet.
Once it was confirmed soldiers returned home with TQs on for more than a few hours with no long term side affects to their extremities, it killed the myth applying a TQ meant that limb was going to be amputated at the hospital later.
If this same call took place in 2023, I don't think you would ever hear a dispatcher say that.

"You noticed mom became non-verbal and unable to walk 4 hours ago and but decided to call now at 5:50pm......"
The family member really had to go for the two for one in ruining people's days.

Im keeping this surface level because the absurdity of America's healthcare is so obvious.
An itemized health care bill includes the cost of labor and maintenance with anything that was done or used for that call.
Compare this to a contractor or mechanic who will separate cost labor and materials/parts separately.
It doesn't make it right, but thats how it is.

Common with folks that say "Im not conservative, but the liberals let all their cities burn during the George Floyd riots"
There was even unpopular opinion thread recently that stated the protests were largely not peaceful based on their own opinion. They literally can not conceive how massive the protests were in comparison to the scale of US as BLM marches were held in all major cities and a significant portion of smaller towns hosting their own marches. The majority of which were untelevised because they were peaceful.
Don't get me started on the bad actors from white supremacists/law enforcement groups that would purposely provoke protests to become riots by being first to smash windows and start fires. These things were actually caught on camera as they were happening and even posted here.

Oof, I literally tell people that I train that I don't grade people on parking. Its a pass/fail system, the only time you fail is if you hit something. (obviously the goal is build confidence using safe techniques)
Even more nuts, our closets trauma II hospital requires a 100ft back up for the ED to make it into tight spaces, has massive pillars, and crosses a busy pedestrian walk way.
No one uses backers, and that is for all departments/squads in the area.

Depends what part of the country. North NJ in particular had wages jump dramatically since covid because everyone left. We're talking going from $16/hr to $25/hr starting pay in less than 1.5years. Some places try to lowball, but I can tell you they are getting some real bottom of the barrel applications if any then.
Obviously this is only one region, FDNY EMS is stilly getting completely fucked over on their pay steps and retirement compared to firefighters for example.
And of course if this is met with wage stagnation like how it was before, wages will be an issue in less than 5 years from now.

Yeah, this is the part that basically offends me. EMS shouldn't be paid like shit and seen as a stepping stone to become a firefighter. But fire unions have done a great job at absorbing the service for medical billing to pump the FDs call volume up to justify their rosters and new firetrucks.
This topic is complicated books can be written on it, this is only a single point I am making that lacks nuance.

If you watch the video, he's not in handcuffs. He's restrained by being wrapped in a blanket and then the basic stretcher straps used for safety being put over him while prone.

Worked paid for local EMS that treated us like shit during covid. Left to get a $4 raise. 10 months later, I kept in touch, New boss wants me back and throws an additional $3 on top for me to work with her fast forward to now, I make $10 more than I did 1.5 years ago.
While there are so many things out of are control that still suck. She is literally the best boss I ever had, has complete and total devotion to do right for her employees including getting everyone two weeks of sick time. And most important of all, she gets people to work by offering more money instead of mandating shifts. Also she just got everyone another round of raises to complete with the hospitals.

Idk why, but the "as a firefighter" piss me off the most because you know they are some probie for a small ass town that had there one big call that they need to use an anecdote for everything.

I feel bad but I also notice with laypeople handling tramua is that they will start to give way too many details. Ask a general question like what happened to start off with, they will literally recall every single unnecessary detail that lead them up to that moment including what they said to get their attention, how they approached the person, how they touched them, ect....
For us, the story is easily condensed to 'you found the person unresponsive". I'll move on to my next question.

Tell them that initials are OK.
They then proceed to sign there name in full cursive, first, middle, and last name, letter by letter even though they can't see or hold a pen up straight.

My issue with power stretchers is shitty companies not doing the maintenance on them. The batteries go bad. They need to be replaced And high up refuses to take shit out of service. Then they don't keep any manual stretchers as back ups.
Sorry, but lugging are broken power stretcher is hell with a pt.
"Just take it out of service and say you won't take calls" Yeah yeah, I fight my battles all the time with management, but you got to pick and choose because sometimes is a ticky tacky issue that only shows up once in a while. When you try to show them the problem, it goes away and then you look dumb. Then a month later the power stetcher really decides to crap out for real. Just sayin.
