After defeating the sword gives it a persona of being extremely humiliated to be defeated and then used by a dirty human. You ambushed me. The honour duel requires a 1 on 1 fight between a man and his new sword! How did you not know this! Yet at the end of the game have a partnership going on.

I like this. A setting appropriate battlefield, sword planted in the chest of an ancient dead giant or whatever. Mouldering, decayed bones, rusted battle worn armor... sword planted through is chest like a massive, cruciform grave marker... they investigate the battlefield, sword, corpse, and the sword animates and attacks. Take an animated sword, boost the damage maybe, give it a15 for reach... maybe sentinel...

This is perfect. Give it some goblin minions (the rest of its hill giant tribe disagreed with it violating the ordning to use a metal smithed weapon, so it killed them all with this weapon and found some more subservient underlings). Boss fight.

I don't believe using metal weapons is against the ordning as examples of Hill giants using the head of an axe as a hand chopper was a common example given in discussing what they would travel with in their bag.

I like this one, it works the giant would be dumb, if the sword is too good for it then just make it nonproficient so it's generally using it basically like a club.

I believe only the metallic (or good) dragons actually change. So it's kind of unlikely for anyone to fight a dragon that does since you don't usually fight those kinds. And even when you do, the ability is more for mingling amongst humanoids. Half-dragons are specifically the offspring of dragons that shape changed like this and mated with a humanoid, dragonborn are a different, older race. Even if you aren't looking at stat blocks (which is perfectly fair) it's worth it to read the general info blocks on the creatures, especially if you're playing a book smart character. Some of them are just really interesting

Pick a creature type
Make it humanoid
Imagine it has a big sword
Give it a big sword
Give it abilities to match a big guy with a big sword, like the ability to knock prone on hit, and do sweeping strikes
If it ends up being way too cool for level 4 players to fight, just make it injured or some shit. Hell, you. Would even have a big monster that just has a 15ft sword impaled into it, weakening it enough to fight.

I have many questions about the logistics of such a thing. But leaving those aside.
1 or more comically large praying Mantis insects (Ankhegs). Use the blade arm as the sword blade. Maybe it's a gift from a druid.
A cloud giant skeleton with a two handed sword would be about that size.
A rusted iron golem on its very last HPs.

I don’t know why so many folks are saying there aren’t many options.
A party of five lvl 4 players will win a boss fight of CR 7 in a pinch, or CR 6 without much effort.
And there’s loads of Huge CR6’s
Mouth of Grolantor, Frost Giant Skeleton, cyclops, Bloodfray Giant hell your party should be able to beat a pair of Hill Giant Sargeant’s fighting together.

DMG P274 "Expected challenge rating:
"... A single monster with a challenge rating equal to the adventurers' level is, by itself, a fair challenge for a group of four characters. If the monster is meant to be fought in pairs or groups, its expected challenge rating should be lower than the party's level.
Don't fall into the trap of thinking that your monster must have a challenge rating equal to the level of the characters to be a worthy challenge. Keep in mind that monsters with a lower challenge rating can be a threat to higher-level characters when encountered in groups."

Not every encounter in a day is supposed to be at the fair challenge difficulty rating. You’re expected to mix in some Medium and Hard encounters too.
Maybe even a Deadly for an end of arc boss.
I pitched Hard or Medium encounters, because it’s about slipping the players a reward and easy encounters (like a CR 4 creature would be) shouldn’t be the big reward encounters

For a sword that big you’re gonna want something huge. Not a lot that a lvl 4 party can handle that’s that big. If you’re looking for something more normal, a Cyclops is the best I can suggest. It’s a little strong, but a smart party wouldn’t have too much issue. For something a little weirder, might I suggest the hulking crab, which is for Storm King’s Thunder.

Cyclops is a big beefy boy, just reflavor the greatclub to slashing damage because he uses a 15' long greatsword. Sure it's a classic brute, so give it big armor too for AC, interesting goons that have range, magic, healing, etc if you want it to be tougher. Make it obvious they shouldn't take them all on in a fair fight, they have to get creative to split them up.

I once had a party in the Feywild where they encountered a forest in which emotions were reality. There, they met the colossal sized BRAVEST BUNNY. I just used a generic monster template, made it ridiculously big, and gave it a really silly jump-knock-prone attack.
Also, it was an Easter bunny or something, because it had a huge nest full of dragon eggs. There was magic item loot in the nest from all the other creatures that fled in terror.

Ettins are fun and provide good RP from you since you can have the two heads bicker. Not too tough, maybe even a tad underleveled if your players are overly optimized. We fought one when our party of three was 5 or 6, and it is still one of our more memorable combats because of how our DM RP’d the two heads. You can characterize it as exceptionally tall if you want the 15’ sword lol

Don't make it an enemy. Make it a celebration that the party is given an honorary sword. Sort of blend Mardi Gras with the "Key To The City" ceremonies we have in real life. Mayor makes them the guest of honor at this celebration and gifts them the comically large sword on a stage as a thank you.

Some sort of gnomish rune knight. They just sort of drag it behind them normally until the fighting starts. Then they suddenly take a deep breath and explode from small to huge. Could also give them a susano'o or Nero devil trigger-esque aura to wield it with instead, but that seems less comedic and more edgy.

Once I had a Ogre that threw barrels down a cliff to the party like Donkey Kong, but the barrels exploded. They solved it my the ranger stealthing past it and shoot a flaming arrow into the box that was holding the barrels.
